Azure Village is brought to you by Marymead.

In order to provide you with your services, Marymead will need to gather personal information about you and/or your family. Your personal information will be handled and protected in accordance with the Australian Privacy Priciples.

Marymead wants you to feel comfortable sharing your personal information so it is important you are aware of your privacy rights.

  • Your personal information is kept private
  • Marymead only collects information that helps us to provide a service to you
  • All client information is stored securely
  • Your information is only seen by those in Marymead working directly with you on a ‘need to know’ basis
  • Your information will only be shared with other services, or another Marymead program, with your permission
  • You can request to see your information. It will not include information about other people
  • If you believe your right to privacy has been breached, you can make a complaint

What personal information is collected?

In accordance with privacy laws, Marymead will only collect information about you and/or your family which is relevant for the direct provision of services. This includes:

  • name
  • age
  • contact details
  • marital status
  • family details including personal information about an individual’s next of kin
  • other basic information
  • health and medical details
  • financial details
  • notes from conversations, phone calls, meetings, emails

We collect personal information that you supply to us to assist you with your general enquiry or application process.

 Collection and storing of personal information

Client records are stored securely in lockable filing cabinets, and on a secure database and server. Marymead also uses secure archiving facilities. Access is restricted within Marymead and to those who need to access the information in providing service to you.

If there is a need to share personal information with another service or another Marymead program, we will seek your consent. You have the right to withdraw your consent at any time.
